Mask Artist Agreement & Statement Form

Please thoroughly read and fill out this form before turning in your completed mask.
  • By submitting this agreement, I agree to all the terms and conditions listed below. Reference herein to "FCMOA" shall mean Fort Collins Museum of Art, a Colorado 501(c)(3) non-profit corporation. References to the "Committee" shall mean a committee formed by FCMOA acting with the authority of that body. I acknowledge that FCMOA is not liable for damage or loss of materials submitted. 1. Artist has no ownership or property rights with respect to the mask. 2. Artist does hereby assign to FCMOA a nonexclusive license to use the artist's design in connection with the project itself, project promotions, project merchandising, books, photos, videotaping, and photocopies regardless of scale or in any other form. 3. Artist agrees to return the finished mask to FCMOA at 201 S College Avenue, Fort Collins, CO 80524 before Sunday, December 4, 2016 at 5:00 PM. The mask can be delivered to the FCMOA office during business hours any time that it is completed. 4. Artist warrants that the design submitted is an original work of authorship that does not infringe upon the copyrights or any intellectual property rights of others. Artist agrees to indemnify and hold harmless FCMOA for any claim, cost or liability resulting from breach of this agreement by the artist. The committee reserves the right to withdraw a mask from the display and auction if the work is determined to be inappropriate.
  • Please tell us your occupation or the name of your school if you are a student. If you are retired, please list your prior occupation along with retired (ex: Retired music professor). FCMOA visitors enjoy knowing a little about the artist.
  • Please provide a brief artist's statement telling the story of your mask. What does it represent? Why did you do it? Keep the statement to 75 words or less. The committee reserves the right to edit statement for purpose of uniformity and space.
